You may complete the paperwork during your first visit or you can complete it at home. If you opt to do it at home, please make your initial appointment first then  download the client intake and contract forms and complete, sign, and return them below.

 If you prefer to bring them with you to your appointment, please print them double-sided, head to foot. Thank you!
